My provider orders Rocephin as follows: Ceftriaxone sodium injection Seq#1: Strength 1, Dose 1 g. I know that to code this you would use J0696 x 4 units. My question is... Does there need to be some documentation indicating dosage in mg since that is how the HCPCS code describes the dosage? Or, is it legal for the coder to just convert the 1g without any documentation of that being done.

CVA. Nurses/CMAs need to document dosage and route. Please consult your company's guidelines regarding this.
