how many ROS are required on a pediatric well check?
 
The history of a well-child exam is not determined as in a problem-oriented E/M service. The ROS would be age and gender appropriate as opposed to that described in the documentation guidelines for other E/M services. Good examples of the preventive history components are found in the Bright Futures templates for documenting well-child visits. An example is found at https://brightfutures.aap.org/pdfs/Documentation Forms 28/2-5 Days Documentation.pdf. Note that the caregiver would be asked about the infant's eating, voiding, sleeping, etc. to achieve pertinent review of constitutional, GI, GU, etc.
