If a provider discusses certain lab results with a patient, or goes over the results of an xray, can those count towards ROS as long as they're not being counted towards anything else? Also, if vitals are documented twice, could one count towards exam and one count towards ROS?

Discussion of test results and vital signs don't meet the definition of a ROS. Per CPT and CMS, a ROS is an "inventory of body systems obtained through a series of questions seeking to identify signs and/or symptoms that the patient may be experiencing or has experienced." These are subjective statements of the patient and part of the history, quite different from the objective measurements of vital signs, labs and x-rays.
