A new pt comes in and fills out our ROS paper form we scan it and the Doc dictates that he reviews the form but does not physically sign the form. Does his electronic signature for the note cover all scanned forms as well? Thanks for the help grandmacora
 
hmmm....is the ROS form dated? I would say if he said he reviewed it you could use it. As long as the form is atleast dated to match the encounter, and make it identifiable (connected) the that day.

I agree with Linda, the form should be dated and initialled by the provider. Our offices use forms like that and their is a spot for the provider to sign and date as well.
