Since the ROS can only be obtained from the patient and if the patient has dementia, etc. can we count this as a complete ROS. We have been told in the past that this is correct, but after the information was given at the lunch time E/M teleconference we can not do this. Please help.
 
We allow a complete ROS if the Provider tells us WHY he can not get a complete ROS. We do not allow simply for unable to obtain. Our coding has held up for several audits over the years so we must be doing something right! Hope this helps.
