Patient is seen as an inpatient consult. When obtaining the ROS, provider states: Difficult to obtain given language barrier but patient does complain of some mild suprapubic discomfort. Should this only count as 1 or does this count as complete since it was difficult to get?
 
I would give it a 10+ ROS based on the statement about the difficulty to obtain due to language barrier. As long as the documentation states the
reason the ROS is not obtained because of .......
