My provider states "family history and review of systems are completed with the patient and documented on the patient information sheet". He is referring to the intake form the patient completes upon arrival to the office. Could this accurately count as a "review of systems"? I personally think it is iffy. Any advice at all would be appreciated... Thank You!:o
 
Yes it could count

Yes, it could definitely count ... BUT ... I would encourage the physician to indicate the date it was completed. Simply adding "today" at the end of his usual statement would do it (assuming it is completed for each visit).
