Hi,

My doctor saw a patient in the hospital, in the history it states that "He has a history of severe demontia, therefor, cannot give any actual history." In the ROS in just states "Unreliable". Can I use this for a ROS since there is a reason for not taking a complete ROS? Thank you.

Yes you can. That would fall under the History caveat.
