RPM Codes 99457-99458 Reporting Requirements - 30 DAYS OR MONTH!

can we bill 99457 (and add-on 99458) again in the next calendar month, even if only 1 day has passed since the previous month ended? I reported a 99457 on February 25 and again on March 20, received a denial from insurance for frequency, stating that 99457 is already reported on a previous DOS on 02/25/2025. Please advise what is the right thing to do? My understanding is that these codes are reported for a month, unlike code 99454 that is reported for a 30 day period.
 
CPT guidelines stated we should Report 99457 once each 30 days, regardless of the number of parameters monitored. In your claim 23days only completed (I reported a 99457 on February 25 and again on March 20). so we should not report 99457. The expected date to report 99457 is March 27th 2025. Hope it helps.
