RPM date of service .....Where to find any CMS / any MAC guidelines?
99453, 99454: Billing Date of service the (when CPT 16-30 transmit days is met) equate to the "date of education or device delivery" or "date of first upload" or "date of last upload"?
99457, 99458 (time based cpt met 20 min, add 20 min) follow Home Health PDGM 30-Day Period (ie: HHC, TCM , CCM) date of service when completed or recurrent at "last day of billing month?

FL MAC FCSO- has no guidance, CMS / HHS / AMA provides no guidance on billing dates of service. Please comment, thank you in advance.
 
From what research I have completed and from the experience utilizing those codes, we use the date of service that is 30 days from the first action. For example, the first RPM read was on 5/1, we did 19 reads that month, I bill on 5/30 for date of service, since this covers the 30 day period. We have not experienced any issues or recoups after record review utilizing this method.
