Our office is looking into reimbursement for evaluation and management of blood sugar data reported to us by our patients via phone call or portal message. We have been researching the RPM codes 99457, 99458, 99453, 99454 but I am unclear on a couple of things:
Can CPT 99457/99458 be reported when the patient reports their data to our office via phone/patient portal messaging? Or do these codes require a device that transmits recordings to the office?
Can CPT 99457/99458 be reported if we are not also reporting 99453 or 99454?
Can 99453 be reported if setting up a device that the patient purchased themselves?
