Rx drug mgt. versus acute acute uncomplicated illness

When a new problem is documented and a rx drug is ordered how many of you include this under moderate decision making on the table of risk. Often a patient will have nausea and vomiting which falls under 2 or more self limited or minor problems for Low risk level. However when a prescription for Phenergan or Zofran is ordered do you choose Moderate risk? I was not sure if this violates medical necessity I was always shown that you select the highest risk level.
