Can anyone point me to a code for billing (azithromycin) 250 mg and 500 mg Tablets my NDC is 68180-0161-06 -azithromycin monohydrate.

J8499 - prescription drug, oral, nonchemotherapeutic, not otherwise specified

As an FYI, you may receive a denial if billing this to a medical plan because self-administered medications are usually covered by patients' pharmacy plan benefits, not medical.
