S0311 Medi-Cal limits

Hello,
I am looking for a source in Medi-Cal that states S0311 has one limit per calendar month. I realize that the code description has per calendar month in the title. The only documentation I could find from Medi-Cal was the ‘TAR and Non-Standard Benefits List: Codes R0000-S9999’. The code is showing as non-standard benefit code however we have a hard coded contract agreement for the code to be reimbursed.
Please help.
