Can CPT 64581- Incision for implantation of neurostimulator electrode
array, sacral nerve (transforaminal placement) be billed bilateral if they are placing electrodes on the RT & LT S3 nerve? I code for an ASC.

Medicare paid it but I want to make sure I am coding it correctly.

Are you talking about the permanent implant? 64581 is for the permanent implantation of the device. We do them in our Urology ASC also - and in 5 years I've never seen them do the permanent on both sides??? The trial, 64561 is done bilaterally as they are trying to determine which side will provide the best results. We are currently being told by the implant rep that PNE's (trials/64561) the ASC facility fee can be billed bilaterally - not just the physician's charges. Anyone heard of this? Thanks!
