I am having a weird thing with Noridian where my billers cannot get 45381 paid for a saline pillow lift, but they can get it paid for a tattoo (like india ink). Has anyone else experienced this? Did you find a work around? They deny 45381 always initially as inclusive to the polyp removal, but upon appeal my billers can get it paid ONLY if it is a tattoo, not a saline pillow. Thanks y'all 
PS -- Per my billers, they have appealed to the highest level for the salin pillow and were told it is inclusive still.... I kind of wonder if they want the 45390 code, but my understanding is they specifically must state it was endoscopic mucosal resection for that one. SO...... thanks again
