Our APRN did an extensive workup on a patient with abdominal pain at the end of the visit it was decided to call ambulance and go to emergency room and our doctor was on call and did another workup-same day. How would something be coded?

You can only code for one encounter, however you can use the complexity from the office encounter and include it with the physician encounter.

so the dr notes of his workup was level 4 but the office workup qualifies for level 5-i can combine the two notes for dr? thanks for your help

You can as long as he references the ARPNs documentation in his note, this then comes under the definition of a shared encounter.
