Wiki Same day hospital charges w/echo

I have a pt that is on medicare, who was in the hospital for a few days. The pt had an echo done on one of those days. Medicare denied the echo because it was done on the same day of a hospital visit. The procedure was done, why would'nt they pay for it, if it was medically necessary & had an approved dx code? How should I process those claims? Please help.
