Same day service - Seen in office, sent to hospital

Hello,

A patient is seen in the office and Physician A sends the patient to the hospital on the same day. At the hospital Physician B from the same practice and specialty sees the patient for treatment and the patient is admitted by the admitting physician (not a part of our practice)...

1. Would the date of service for that day default to the inital hospital visit billed under Physician B?

2. Or would the date of service for that day default to the office visit billed under Physician A?

Dr. A could charge for a E&M service with the appropriate supporting documentation.

What type of treatment did Dr. B do? If Dr. B anything other than an E&M, he could charge for it. Dr. A would want to add a modifier 25 to his E&M since the billing would come out of the same office.

Dr. C would bill the initial hospital code (admit).
