A patient comes in and see one of our PAs and had an office visit and AK treated. After that visit the patient sees our physician and has an excision of a lesion. Codes for PA are 99213-25 and 17000, 17003. Physician codes are 11602, 12032. Would I put modifier 24,25 on E&M and 79 on the AK's?
