Same drug pushed twice

If we pushed 5FU twice during a chemo treatment do we code 96411 or 96411 X's 2. I thought it should be 96411 since it is not a new substance. Any help would be appreciated.

If the drug was the only drug given that day and it was administered twice ( two seperate times.) Then you could bill 96409 IV push initial and then bill 96411- IV each additional push. 96411 is not an initial code therefore it cannot be billed 96411x2 . If other chemo was done then yes it can be billed twice( administration) like that but add the dosage together.

Is it the same concept with 96417?

A chemo drug (non-initial) was infused at two separate times during the SAME encounter. Once from 1:00-1:30 (30 minutes) and then from 2:43-3:15 (32 minutes). Would it be correct to bill 96417 OR 96417x2?
