Wiki Same injection codes with different NDC's

When we give a Rocephin injection of 750mg the nurse uses a 500mg and a 250mg. These have different NDC codes. I tried billing two separate lines (so I could report the different NDC) with a modifier 76 and was denied. Any suggestions on how this should be reported?
