Sanity Check On CRT-D Gen Replc with LV lead exchange

Hey there,

Sorry my header says CRT-D but I meant CRT-P.
Patient presents with CRT-P 3 lead system with gen at ERI.
LV lead exhibits macrodislodgement.
Provider replaces generator, extracts LV lead, and replaces with new one.

How would this scenario be coded?
