I have a report which states "scar revision" which usually is 13xxx for complex repair with no reporting of excision of lesion codes because it is included as per CPT guidelines but in the body of report this is described as a layered closure so wouldn't I then be able to code both excision of lesion code 114xx and intermediate repair codes 12xxx?
 
Layered closure + lesion excision

Based on what is documented ... yes, if you did NOT have a complex repair, you can code the excision of benign lesion plus intermediate repair.
