How would you code out a colonoscopy that started out as a Screening Z12.11 and a biopsy was done for a diminutive rectal polyp, but the path results show that this is a condyloma acuminatum/anal wart (dx code A63.0). This is not a covered diagnosis with Medicare and will most likely be denied if I code out 45380-PT w/ 1)A63.0 2)Z12.11. Is it legal to not code out the biopsy 45380 and just code out G0121 w/Z12.11, since the results were negative for rectal polyp??
