I would use ICD-10 code Z02.1 - Encounter for pre-employment examination.
Exams or tests done for employment clearance or other administrative purposes are usually not covered under medical plans, so this is something that either the patient or the prospective employer should be paying for rather than being billed to insurance.

I would get an ABN/waiver, because it will, most likely not be covered. But perhaps the employer will pay for it.
