Wiki Scribe verbiage EMR VS paper??

Does anyone have documentation of the rule of verbiage when using EMR. Is it enough to just have the scribe named marked as medical staff and having the dr sign off or does there need to be special verbiage entered in the record like it was on paper?:confused:
