Indication for operation: Pt is s/p incision and drainage of scrotal abscess, now with recurrent
arterial bleeding from the wound

Operation performed: Wound exploration with fulguration of bleeding sites
Description of operation: After informed consent was obtained, he was brought back to the OR
and underwent GETA. He was placed in low lithotomy position. His genitalia and perineum was
prepped and draped in the sterile fashion. A timeout was performed
The wound was inspected and he had arterial bleeding from the posterior aspect of the perineal
wound. Dissection was carried around the suspected bleeding site and was fulgurated using
bovie cautery. There were venous bleeding from the posterior/lateral aspect as well that was
fulgurated. The wound was irrigated and after 5 min of observation, there was no further bleeding.
The wound was packed and he was returned the supine position, extubated and taken to the RR
in stable condition.
Plan: continue wet to dry dressing changes starting tomorrow. Monitor H/H.
