Wiki Second provider completed addendum

I have a scenario that I would appreciate some thoughts on.

I have Dr. A who completes a note but leaves out a date of service, a diagnosis or time and I have Dr. B, who is in the same group practice, who adds an addendum with that information. Is this ok to be billed under Dr. A?

The documentation I have found regarding addendums doesn't specifically mention it must be the same provider who did the initial documentation but I can't help but think that the addendum should be completed by the provider who did the original documentation.

These both are psych providers in the same group practice.

You're right in saying that the addendum should be made by the original physician who completed the original documentation. Even though Dr. A and Dr. B are of the same practice, the original documentation needs to be ammended by the orginal physician.

Thanks for your input Annamarie. Could you point me in the right direction of a guideline that states that is should be the same provider as I would like to present this to these providers.
