Second Site for IV Infusion

Patient came in for chemo which was facilited through their port. Patient received a peripheral line in the other (left) arm to give magnesium so we were giving infusion in TWO separate sites. The magnesium ran for 2 hours. Would the appropriate codes be with a -59 modifier on the 96365-59 and 96366-59 to show it was from the second IV site?
 
I am thinking if two separate lines were used, you can use 2 initials, such as 96365 but it has to have a 59 modifier. Check your CPT book, page 518, paragraph 2.
 
No. I would not put a 59 on the 96366. You should code it like this.
