Does anyone have a template that they could share for helping to capture secondary diagnoses to help support medical necessity when billing for benign lesions?

I am not sure what you are wanting. If the provider had a suspicion for malignancy prior to removal and the path report states it is benign, we always used V71.1 first listed as it is first listed only allowed, and the benign DX code secondary.
