Here is the information that I have available from the report:
Semen analysis regards to requistion ....
-Semen Volume: 2.5 ml
-Sperm count 3750,000,000/ml (high)
-Semen pH 8.5 (high)
-Motility PR: 70% (HIGH)

SPERM MORPHOLOGY

Aprpoximately 95% normal forms are identified with no kinked tails for immature forms. No hamster egg forms or coil tail forms are identified. Essentially normal sperm morphology seen with double head and double tail appearance and vacuoles are also identified.

How do I code this out? Any help appreciated

I d use the CPT 89322 seems to cover all sperm analysis. See page 696 in CPT manual year 2022. Ensure link to proper dx code to get funded properly. Possible dx can be N46, N43.4 or Z31.41 or Z98.5 per provider s notations or order for sperm testing.
