I work for three surgeons. Two of them have great documentation and always document a separate note for an ultrasound that was done here in the office in order to bill it. My third doc - not so much. Isn't it a requirement that there must be a separate note dictated for the ultrasound in order to be billable? I have looked through the DG's and can't find it. Does someone have something in writing or a site that they might be able to direct me to.
