Hmmm...so, patient has a history of CVA, and on follow up office visit, provider documents weakness in the right lower leg, in the Physical Exam only. Is it presumed to be a late effect from the stroke, or does the provider have to specifically state the condition is a sequelae of the stroke.

Appreciate any guidance offered.
