I need a little assistance with Radiation Therapy Procedure Coding for Bone Mets derived from Prostate Cancer.
The patient has 3 separate bone mets sites. Patient came in separately for each area.

For Clinical Treatment Planning Note, can we only bill one of 77263?
Each site has a separate Initial Simulation 77280 - can I bill one for each site?
For Treatment Planning - there are qty 3 "3D" plans - I billed qty 2 77295 as they have separate data sets and I believe I can bill the 3rd plan as a "2D" - is this correct?
Can you please verify if the Dosimetry 77300 is still included in the "2D" plans 77307/77306?

Please advise........I would really appreciate it!!
