sequencing primary & secondary cancers

If the encounter equally treats both primary and secondary cancers, is there a sequencing rule?

According to some payers they require a certain sequencing per the drugs being given for treatment. As far as coding for E&M, i would have the primary cancer first and then metastatic sites following after.
