Wiki service facility location addresses when going to pt home

so after 2 years a debate has arose in my office

i work in a private providers office (pos 11 / cms-1500 form). the providers see patients in their homes, alfs, snfs, rehabs, etc

we have been using the address of the practice we work out of and then the pos code for the service location. so say we see a patient in their home in "smalltown" but our physical practice is in "bigcity" the question --
should we have the "smalltown" address in box 32 (service facility location) or our "bigcity" address

i know the pos code will be 12 / 13 / 31 etc...

So if it is home, your big city is fine with POS 12. When you see patients as "out clinic" at another office/facility location your billing info in box 33 should be your big city, box 32 service facility should be your actual service location.
