Hi,
Our provider will lease space from another practice. What information do we put in box 32 of the 1500 claim form? The name of the practice is under my employer, she will not be an employee of the practice already in existence. . She will have her own NPI, TIN and contracts with insurance companies.
How do I do this?

Box 32 is the physical address where services are rendered. It does not matter whether you own, rent, lease or sub-lease. It is the actual physical address and is used to determine locality for Medicare (and may impact fee schedule for commercial payors as well.)
If you wish to limit the amount of mail/payments/EOBs received at that address, you may want to consider setting up a PO Box or other location as your billing address. Some carriers and patients will still make the mistake and mail items to the service address, but it will limit the amount of mail to potentially get mixed up.
