Am I mistaken, or are procedures such as injections, infusions, hydrations, etc, no longer bundled into the ER CPT codes, and billable along with the appropriate E&M?

Bill them on the facility side, not pro side.

Correct they have always been the facilities charge which is why the physician does not bill for these. The facility will use a 25 modifier on the E&M with the infusions.
