I have a counselor who is seeing a patient that has been assaulted as a child she is using T7422XS she is an adult now I would think she should be using personal history of abuse, sexual am I correct?
Sequela has no time limit , however there needs to be a cause and effect relationship documented. Sequela indicates there is some residual condition related to the event, in this case the sexual assault. For instance PTSD due to the assault would be coded with the PTSD code first and the assault with the seventh character S secondary. If there is no residual effect then it would be more appropriate to code the assault as history.
if the patient was assaulted as a child but now is an adult would the T code be coded as an adult (T74.21XS) if the patient is being treated for PTSD because of the assault.
