Can someone go into detail as to what entirety means since 95/97 guidelines are not used. What documentation would be needed to justify entirety??

If you're referring to history or exam CMS has not issued any information on how it will define or audit the terms beyond referring to the CPT guidelines that call for a medically necessary history and/or exam when performed. It really is very unclear.
