Can someone tell me when billing for a shared/split visit in the hospital using the EHR if it is acceptable for the doctor to state "Authored: Plan, Outpatient medication profile, chart review. Co-signer: Plan, Consulting services/Reason for consultation, chief complaint/history of present illness, past medical history, family history, social history, allergies, outpatient medication profile, physical examination, chart review. And the P.A. does the same as Authored and what she did and bill it under the doctor or does the doctor have to do a separate report attached to the P.A.'s in order to be able to bill it under the doctor?
