I am confused as to if/how to code for a open RC Repair when the Surgeon attempted to repair the Rotator Cuff but after attempting it was found to be irreparable. Any information would be extremely helpful and graciously accepted.

23412-52? Would really need the note for this. There is no doubt that debridement was done of the tendon(s) before the repair was attempted. Was a SAD done with an acromioplasty? That would be 29823&29826. Was more than one tendon to repair? Chances are that the physician performed some billable procedure(s) before attempting the repair. Post the procedure part of the note.
