Hello,
For 2010-2011, we have been billing the 27096-RT or -LT and 73542-26. Then Medicare is requesting a refund for the 27096. Can anyone tell me what the problem may be? We always use diagnosis codes listed on the LCD as covered... Denial states not medically necessary... Is it because no 77003 was billed? It is bundled with the 73542!
Please let me know if there is any info that may be helpful!
