How are you handling the documentation for a preventive visit and a sick visit on the same day? Two separate notes or combined into one note, but clearly documenting each visit?

To code the sick visit along with the preventive the problem pertinent History, ros and PE should only be considered as for preventive exam they will do a comprehensive evaluation
