If a patient came in as a walk in due to issues with their SP cath and ONLY saw an RN. Who assessed the patient and documents that there is granulation tissue that they treated with silver nitrate. Can it still be billed? Does this fall within the scope of practice for the RN or does this need to be ordered by the provider or PA?
 
This is much more a compliance concern than a coding question. MY understanding is that RNs are not permitted to determine the patient's treatment plan. They may follow the instructions by the MD/DO/PA/NP. If you are concerned the RN is working outside their scope of practice, your state's licensing board and nursing associations are good references.
What code were you considering billing? And did the physician/ACP provide general supervision or direct supervision - in other words, were they present in the office and immediately available but just not in the room?
