Simple Mastectomy with removal of Ruptured Implant

If a surgeon performs a simple mastectomy, 19301, and also removes a ruptured implant, 19330. Would you code only the simple mastectomy or both procedures? My thinking is only the mastectomy.
A different surgeon replaced the implant at the same session, 19340, and also did the closer, would the 80 cm adjacent tissue transfer be coded 14301 & 14302
Any assistance would be appreciated. Thank you.
