Wiki skilled nursing facilities and e/m/ codes

I have the codes (99305- 8) for the SNFs, however, it can be hard to determine which is the correct code, does anyone have a template or flow process that they use to determine. The ones I have are for hospital or office and I know the
levels of care are determined differently for SNFs. Any assistance on this would be greatly appreciated. Thank you
