I am probably making this harder than it really is but....
A elliptical incision in order to remove the skin lesions. There were 5mm lesions noted on the anterior-posterior neck skin lesions. These were completely removed. Next, provider went through the subcutaneous tissue into the deeper aspect of the neck. These were app 6-8 small circular nodules measuring 5mm. This completed the R neck mass excision. Next, the site of the lipoma was identified and freed from surrounding tissue. R neck lipoma. This was closed in several layers with 4-0 chromic and 3-0 chromic for the deep and subcutaneous layer with derabond for the epithelium.
I am wanting to use 21556, 11421 OR 11420, and I think another code should be use but I can't figure out which one.
