Claim denied CPT 11200 stating inclusive; CPT 11201 denied not covered unless a pre requisite procedure/service has been provided.

Any help?
 
11201 won't be paid unless 11200 is paid. And since 11200 was denied because it's included in the E&M, they denied both.

11200 is a skin tag removal, and generally considered cosmetic. Both of your diags are non specific, (and not skin tags) so I'm betting it was denied also for medical necessity.
